The Transatlantic cruises are consistently some of the longest Disney offer. They range from 10 to 16 nights, when heading west they leave from the UK back to the USA. When heading east they usually end in Spain.
They share much of what they offer with the European itineraries which means you shouldn't expect fireworks and Halloween on the high seas. As many Disney ships are built in Germany they will travel the transatlantic route though may not be bookable cruises. Many ships will also travel to Europe for dry dock.
The vary their ports of call regularly so there is no guarantee a port will be repeat the following year. Itineraries could feature any of these ports (and possibly others in the future):
